Georgia Milestones Assessment School Inquiry Form
The Governor's Office of Student Achievement has identified schools with classrooms requiring further inquiry through the erasure analysis and the unusual gains and unusual response pattern analysis of the 2017 Georgia Milestones Test. This form is to be used for district feedback of individual schools requiring further inquiry identified in the Governor's Office of Student Achievement's findings for the 2017 Milestones EOG and 2017 Spring EOC comprehensive analysis. This form provides LEAs with schools requiring further inquiry an opportunity to explain the reasons for why specific classrooms were flagged for wrong-to-right (w-t-r) answer changes, and/or unusual patterns and gains identified by the testing vendor. The form should be filled out and submitted by district level personnel.
*Note: A separate form must be filled out for each identified school, and if a school was identfied for both the answer change and unsusual response analyses then #9A and #9B should be answered for that school.
Forms must be submitted by February 19th at 5:00 PM. Please fill out a separate form for each identified school. All answers must be entered in one session, so it is recommended that whoever enters the LEA response gather answers to all questions prior to entering information into the form.
Once received, GOSA will review it in conjunction with relevant state-level data to determine whether the inquiry can be closed or if further information is required, which may include a document/records request, full investigation, or an on-site audit. GOSA will notify the LEA Superintendent and Director of Assessments by email of closure or next steps by May 18, 2017.
Form responses are confidential. Only GOSA Academic Auditing and GaDOE Assessment Division will view submissions.
